CVSS reports are available in pdf format (viewable with Adobe Acrobat Reader software, available as a free download at www.adobe.com). Electronic distribution of reports is strictly limited to 1) the referring hospital; and 2) the pet owner of legal record, as indicated in the referring hospital's medical record.

Documents available for electronic receipt currently include:

	Operative reports
	Recommended Care Instructions

Requests to receive a copy of one or both of the above reports for a particular patient may be made by sending an email to reports@cvssweb.com with the following information:

	Name of pet (as indicated in medical record)
	Name, mailing address (as listed in medical record) and email address of person requesting the report
	Date of procedure
	Type of Report requested (Operative and/or Care Instructions)

Information contained within a Report Request is verified with the referring hospital's medical record so as to ensure proper delivery of reports and confidentiality of medical records.
